Why do I need to complete a health form?

 

As this is a prescription-only medication, we need to ensure you are suitable to use this medication. The health form is all about you, your history and your goals. Your information will NEVER be shared with any 3rd party. We only share your information with our pharmacy partners, who dispense your medication.

PLEASE NOTE: We will require a form of ID to verify you are over the age of 18 and that we are prescribing for a real person. If you have used the medication before, we will require proof of previous usage. If this information is not loaded with your health form, it will cause delays to your order or a refund to be issued.

When will I get my package?

We use DPD, APC or Royal Mail Special Delivery due to the items needing to reach you ASAP. This costs £4.99 and tracking information is provided. Your item will need to be signed for by someone at your delivery address. You can choose a delivery day to suit your schedule. The item is usually sent the same day it is dispensed.

What happens after I have done the form?

Your form will be screened by one of our prescribers and we will contact you with the outcome as soon as we hear. This can take up to 72 hours to process. You can either proceed to checkout to make payment now or, once you have been approved for treatment, we can send you an invoice for payment. Once paid, we will then aim to deliver on the soonest delivery day you have selected.
